dc.description.abstract | INTRODUCTION. Chylothorax results from damage to the thoracic duct due to rupture, laceration, tear, or compression. It is frequent as a complication after cardiothoracic surgery. The basis of treatment has been based on: initial drainage, diet modification, use of synthetic somatostatin or analog (octreotide), surgery, prevention and treatment of complications. OBJECTIVE. Describe the symptoms of chylothorax in pediatric surgery patients, as well as its different treatment approach. CLINICAL CASE. A 4-month-old male patient admitted to the pediatric emergency room of the Carlos Andrade Marín Specialties Hospital with respiratory distress, a history of corrected esophageal atresia in the neonatal stage. He came with difficulty breathing, chest radiography showed right pleural effusion. Diagnostic thoracentesis with cloudy and milky-looking fluid outlet, a right chest tube was placed, initial fast, parenteral nutrition for 4 weeks until resolution of the chylothorax was verified, discharged in stable condition. DISCUSSION. Conservative treatment of chylothorax was based on a series of measures; Chylothorax drainage, initial digestive rest, parenteral nutrition, qualitative modification of the enteral diet, the use of octreotide. In the case of the patient, the aforementioned measures were used and the chylothorax approach was successful. CONCLUSIONS. Chylothorax has been a rare pathology in pediatric age, the main cause has been a history of cardiothoracic surgery, its treatment in most cases has been conservative, multidisciplinary management is essential for success in addressing this pathology. | en |

dc.description.abstractalternative | INTRODUCCIÓN. El quilotorax resulta de un daño al conducto torácico por ruptura, laceración, desgarro o compresión. Es frecuente como una complicación posterior a cirugía cardiotorácica. La base del tratamiento se ha fundamentado en: drenaje inicial, modificación de la dieta, uso de somatostatina o análogo sintéticos (octreotide), cirugía, prevención y tratamiento de complicaciones. OBJETIVO. Describir la clínica del quilotórax en pacientes pediátricos posquirúrgicos de cirugías, así como su diferente abordaje de tratamiento. CASO CLÍNICO. Paciente masculino de 4 meses de edad, ingreso a sala de urgencia pediátrica del Hospital de Especialidades Carlos Andrade Marín con dificultad respiratoria, antecedente de atrésia de esófago corregida en etapa neonatal. Acude con dificultad para respirar, radiografía de tórax evidenció derrame pleural derecho. Toracentesis diagnóstica con salida de líquido de aspecto turbio y lechoso, se colocó tubo de tórax derecho, ayuno inicial, nutrición parenteral durante 4 semanas hasta comprobar resolución del quilotórax, dado de alta en condición estable. DISCUSIÓN. El tratamiento conservador del quilotórax se basó en una serie de medidas; drenaje del quilotórax, reposo digestivo inicial, nutrición parenteral, modificación cualitativa de la dieta enteral, el uso de octreotide. En el caso del paciente se utilizaron las medidas mencionadas y el abordaje del quilotorax fue exitoso. CONCLUSIÓN. El quilotórax ha sido una patología rara en la edad pediátrica, la principal causa ha sido el antecedente de cirugía cardiotorácica, su tratamiento en la mayoría de los casos ha sido conservador, el manejo multidisciplinario es esencial para el éxito en el abordaje de esta patología. | es |
